Thank you for showing interest in our family history. There are many families out there that have need of information that you may have in order to connect to lost family members. Information that will provide clues for many of us to find more information about our ancestry.

In the form below you will have opportunities to document information about your self, your parents, your grandparents, siblings, and children. Following that is a section about your spouse(s) and any children born of those unions.

Above all, it is of utmost importance to have accurate and correct information. This form will allow you to upload up to 4 generations of information. There is a space provided for additional details. If you have a gedcom file that you would like to share, please advise in the comment section and I will contact you by email with instructions on where to send it.

This is a new form and I am tweaking it as I go to make it as easy as possible to complete and send information. I apologize that it is lengthy and cumbersome bu until I find a more simple method of gathering data, I'm afraid this will have to do. I'm sure it will be a bit confusing to some as well. If you have questions or comments, please send them (along with an email address so that I may reply) in the comments section and I will try to help you.

The reason I need so much information is that there are so many names of John, William, and Henry, that in order to place your family in the right line, I need all the information I can get to properly document the lineage. I will also research each name for more documentation before connecting them with a main line of the ancestral tree.

If you are not sure of information, it is better to leave it out than to guess and be wrong. You can make notes about uncertainties in the Additional Details section. We can always add information later and all information that is submitted will be researched and verified so that we can make it as complete and accurate as possible. Once you provide your family information, I will send you a link to the page with information concerning possible ancestral relations for your surname. Please do not share this link with others. My objective is to gather information from families to create a work that can help all families.

If any information is not applicable, or does not apply, please leave it blank. If you are unsure about dates, use abt for About, bfr for Before, aft for After and the year. If you are reporting twins or multiple births, please enter them in order as that is the way they will appear in print.
